Important! Back to School Forms Message from Mrs. Cox Dear St. Anastasia Catholic School Families, I pray that you are having a great summer! We are gearing up for a wonderful 2020-2021 School Year. Each year, we normally send home paper agreements for parents to complete on the Parent and Student Handbook and Photo Release. This year we have decided to have families complete the forms online. We have also included an extra sign off form related to COVID-19. Please complete the agreement forms by entering your child's name, grade/ homeroom teacher, then checking off "I agree" in each box. On the bottom of the form please type your name (this will be your electronic signature) and enter the date. Please feel free to contact me if you have any questions or concerns.
